how to get one-time-token auth working with AD B2C

I'm following this poorly documented and riddled with spelling and grammatical errors document and sample due to the lack of anything official provided my microsoft documentation as usual: https://github.com/azure-ad-b2c/samples/tree/master/policies/custom-mfa-totp

I have the rest api, custom attributes, etc. set up. I have added the user journeys section to the extension policy and uploaded it via custom policies in the portal. However, when signing in or signing up the user, it never goes through the auth process. What could I be missing here? Is there something else I need in order to "activate" the custom user journey?
